Creating, sharing and tidying up forms

Start a form by describing it, from a template or blank; share it by link, QR code, email or on your website; and archive or delete forms safely.

Creating, sharing and tidying up forms

The Forms list

Forms lists your checklists, sign-ups, enquiries and reports. Surveys and calculators have their own pages.

  • Use the tabs to see All (everything except archived forms), Live, Drafts or Archived. Each tab shows how many forms it holds.
  • Search by name with the search box.
  • Each form shows how many responses it had in the last 30 days and when the latest arrived. Forms on a schedule show how often they repeat.
  • The main button on each row is Continue editing for a draft and Responses for a live form. Live forms also have Share. Everything else is under the ⋯ menu: edit questions, settings, open the form, duplicate, unpublish and archive.

Starting a new form

Press New form and choose how to start:

  • Describe it: write what the form should collect, for example "a daily fridge temperature check with a photo if it's too warm". AI drafts the questions for you.
  • From a template: pick a ready-made form such as a fridge check or an end-of-day report. Managers can use templates.
  • Blank: give it a name and add the questions yourself.

Every new form starts as a draft. Nobody can see it until you publish it.

Sharing a form

Press Share on a live form. There are four ways to share:

  • Link & QR: copy the public link, download the QR code as an image, or print a poster with the QR code on it. If you manage links for the location, you can also make a dynamic link that you can point somewhere else later without reprinting the QR code.
  • Email: type one or more email addresses. Each person gets their own email with the link, and Mavos tells you who it was sent to and any addresses it couldn't send to.
  • Website: on a Mavos website, add an enquiry form or calculator block in the website editor and choose the form. On any other website, copy the code and paste it into the page. Either way, the website always shows the version you last published.
  • In Mavos: show the form on the kiosk, customer display or a staff screen, run it as a scheduled check, or send it as a follow-up after an order.

Unpublishing, archiving and deleting

  • Unpublish turns a live form back into a draft. Its link, QR codes and website embeds stop working until you publish again.
  • Archive puts a form away. It stops taking responses but keeps them. If the form is on a schedule, archiving pauses the schedule, and Mavos tells you which schedules first. Restoring the form doesn't restart its schedules; resume them from Checks → Schedules.
  • Delete permanently is only offered for archived forms. It deletes the form and all of its responses and can't be undone. If the form has responses, you type DELETE to confirm.
